Post-Acute, Short-Term Rehabilitation

In our short-term skilled nursing and rehabilitation Pope's Hill Unit, our expertise is in managing the smooth transition to rehabilitation care for patients who come directly from hospitals.

Orthopedic rehabilitation is our specialty; we take a team approach to recovery with our rehab and nursing services for total knee and total hip replacement surgeries.

Some of the features of our program include:

  • A pre-op meeting with our admissions coordinator and discharge planner
  • Tour of the facility and the Pope's Hill Unit, including a typical patient room
  • Chance to meet the medical and rehab staff
  • Pre-registration paperwork completed in advance

For our rehab staff, the patient's healing process is our entire focus. Our team of licensed physical therapists, occupational therapists, speech therapists, and licensed therapy assistants have extensive acute hospital or acute rehab experience in geriatrics, orthopedics, and cardiopulmonary and neurological interventions. We provide a comprehensive program of rehab therapy seven days a week, all with the goal of “getting you home.”

We are proud of our orthopedic rehab services, which are designed to streamline your admissions and discharge process and give you the best chance for a speedy recovery.

Our orthopedic surgery recovery standards are a five-eight day stay for a total knee replacement and 10-14 day stay for total hip replacement. We achieve this by working with you or your family member to develop an individual treatment plan based on “care mapping,“ the planning and charting of daily goals and progress. The rehab staff is led by Rose West, a certified physical therapist with more than 12 years experience.

In our Ashmont Unit, we also expertly treat patients with other medically complex conditions, such as:

  • Stroke
  • Heart attack, congestive heart failure and other cardiac occurrences
  • Cancer
  • Unstable diabetes
  • Chronic respiratory diseases
  • Generalized functional decline (due to prolonged hospitalization)

For all our short-term patients, we offer highly skilled nursing services in:

  • IV therapy
  • Pain management
  • Post surgical care
  • Wound care
  • End of life care

Short stay patients receive extensive nursing and rehab services from a team led by nurse managers. Our focused individual treatment plans are designed to bring you or your loved one to the highest functional level as quickly as we can.

Because we know a positive mindset is a vital component of healing, we provide our patients with varied recreational activities, as well as personal care amenities such as a hair salon/barbershop. We also encourage and accommodate the active involvement of family members in the patient's recovery.

We accept private pay, Medicaid, Medicare, commercial insurance, and managed care health plans, such as Harvard Pilgrim, Blue Cross/Blue Shield, and Tufts, and long term care insurances.

Long-Term Care

The Bostonian subscribes to the philosophy of the Eden Alternative, a not-for-profit organization dedicated to remaking the experience of aging and disability. The focus is on re-imagining traditional nursing homes into non-institutional communities that can eliminate the most common plagues of elders: loneliness, helplessness, and boredom.

We describe our Neponset Unit as a neighborhood and the residents as elders. We have brought in dogs, cats, birds and fish to live in the unit, providing an opportunity for our elders to give meaningful care to other living things. An expanding garden area and indoor plants provide that same sense of purpose. Under the direction of Nurse Manager Kathy Dunn, our goal is to transform Neponset into a habitat for human beings rather than a facility for the frail and elderly.
